Our membership model
Our membership fee is $250, automatically charged every month. We do not charge an initiation fee for new patients; however, we require a minimum commitment of six months. This membership gives you access to multiple telehealth or in-office visits, quick text consultations, discounted labs, imaging, and medications.
We do not take insurance and we are not able to serve Medicare patients.
North Shore Longevity proudly serves patients residing in Wisconsin, Illinois, Indiana, and Florida. To ensure compliance with state regulations, our telehealth services are exclusively available to residents of these states. If you have any questions about eligibility, feel free to contact us.
